ABSTRACT: Fractures and dislocations at the base of the fifth metacarpal are uncommon injuries that can result in poor functional outcomes if they are not properly diagnosed and treated, or present in a delayed fashion. These injuries are the result of high energy, generally an axial load to the metacarpal. Given the rarity of such injuries, and the paucity of large series of reports on these injuries in the literature, there is no consensus on the gold standard for treatment, although many treatment modalities have been reported. The purpose of this article is to review diagnostic and management considerations of fractures and dislocations at the base of the fifth metacarpal, particularly those that present in a delayed fashion.
